Welcome to OneCare Connect

OneCare Connect Is Changing to OneCare

On January 1, 2023, your OneCare Connect plan will change to OneCare, CalOptima Health’s Dual Eligible Special Needs Plan.

Here's what you need to know:

  • You will continue to receive your health care benefits and will not have any gap in coverage.
  • You do not need to do anything to enroll in OneCare.
  • You will be automatically moved to OneCare.
  • You will be notified about the change again by mail in October 2022.

What is it?

OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan) combines your Medicare and Medi-Cal benefits to make getting quality health care easier. At no extra cost, you also get vision care and transportation to medical appointments.

Plus, you get support so you receive the services you need, when you need them. A Personal Care Coordinator works with you and your doctors to create a health plan that fits your needs.
