What is OneCare Connect?

OneCare Connect Cal MediConnect Plan (Medicare-Medicaid Plan) makes it easier to get the health care you need because it combines your Medicare and Medi-Cal benefits in one plan.

How can OneCare Connect help you?

On your own, it can be tough to figure out how to get the right health care. With OneCare Connect, you have support from a Personal Care Coordinator. This person can answer your questions and help you get the services you need. OneCare Connect also offers access to certain home and community services to help you live safely at home.

What services will you receive?

OneCare Connect provides services covered by Medicare and Medi-Cal. At no extra cost to you, you also get worldwide emergency care, dental care, vision care fitness benefits and transportation to medical services. View the member handbook for details.

Are you eligible for OneCare Connect?

Most people age 21 and over, who live in Orange County and have full Medicare and Medi-Cal benefits, are eligible. There are some exceptions. If you would like to find out if you are eligible, call OneCare Connect enrollment at 1-855-705-8823, Monday through Friday from 8 a.m. to 5 p.m. We have staff that speak your language. TTY users can call toll-free at 711.

How do you join OneCare Connect?

Most people do not have to do anything to join. If you are eligible for the plan, you will automatically get 3 notices in the mail before becoming a member. If you want to voluntarily enroll before you get the notices, call OneCare Connect enrollment at 1-855-705-8823, Monday through Friday from 8 a.m. to 5 p.m. We have staff that speak your language. TTY users can call toll-free at 711.

Can you still see your regular doctor?

Yes, you can when your doctor is part of our network which has more than 8,000 doctors. If your doctor is not in the network, you can keep seeing your doctor for a period of time if he or she agrees to work with OneCare Connect.

What kind of support can you get from OneCare Connect?

You get a Personal Care Coordinator that can answer your questions and arrange your health care. You also get support by having a care team. Your doctors, home and community services providers, and Personal Care Coordinator work with you to create a care plan that’s right for you.

Does OneCare Connect let you make choices about your health care?

Yes. OneCare Connect wants you to take an active role in your health care. You can choose your doctor and decide how much you want to work with your care team. You can also change doctors every 30 days.

How much does OneCare Connect Cost?

If you do not pay anything now for having Medicare and Medi-Cal, there is no cost for OneCare Connect. OneCare Connect has small co-payments for your prescription medications.
